Qualifications
- Be a graduate of an accredited school
- Hold an active, unrestricted license to practice medicine in the United States
- Hold a current certification approved by one of the following:
- American Board of Medical Specialties (ABMS)
- American Osteopathic Association (AOA)
- American Board of Podiatric Surgery (ABPS)
- American Board of Podiatric Orthopedics and Primary Podiatric Medicine (ABPOPPM)
- American Dental Association (ADA)
- American Board of General Dentistry (ABGD)
- Have a minimum of five (5) years of independent practice and experience in the applicable board certified clinical specialty with recent clinical experience within the past three (3) years
- Have no history of disciplinary actions
